NEW PATIENTS
All new patients are asked to complete the medical history and patient registration forms, and must present their insurance card(s) upon their first visit with our office.
It is also the patient's responsibility to bring their referral, if required, with them to our office.
We ask that all new patients arrive a minimum of 20 minutes prior to their scheduled appointment in order to complete the registration process. Patients who have already completed the registration forms (by accessing them through our web site or by picking them up at our office) need only arrive 10 minutes prior to their scheduled appointment time.
You can save some time before your first visit by completing your new patient paperwork from home. You will need the Adobe Reader® in order to view/print the forms. You can download it for free by CLICKING HERE.
INSURANCE ACCEPTED
We accept a variety of insurance plans at our office, and we frequently update our participation with new insurance providers. Please contact our office at (906) 225-7707 and we will be happy to verify your coverage.
